ADA Complaint Form

Title II and III of the Americans with Disabilities Act of 1990 (ADA) provide that no entity shall discriminate against an individual with a disability in connection with the provision of transportation service. We will accommodate any reasonable modifications upon request. Please submit any request by submitting any request to our main office via 580-248-5252. All ADA complaints are kept on file for 5 years.
